v3.0.4: fix: gate BYOK cost fields on usage.is_byok (#392)
About SN15 ORO
OpenRouter fills usage.cost_details.upstream_inference_cost on non-BYOK calls too (equal to cost, with is_byok false), so the amount-based rule counted every call as BYOK. A call is BYOK only when usage.is_byok is True; only then does its upstream cost add to inference_byok_cost_usd and the call to inference_byok_requests.
